Foothills VNA uses a simple device that allows our patients to monitor their blood pressure, pulse, temperature, weight, blood sugar and blood oxygen levels from their own homes. The frequency of the testing is determined by the patient’s status and they and their caregivers are taught how to use the equipment. The information is then transmitted back to our office where our nursing staff is able to monitor these readings to assess for abnormal readings. Based on the transmitted data, the nurse will either call the patient or make a home visit where she can do a physical assessment and report back to the patient’s physician. By assessing the patient frequently via the telemonitors, changes in the patient’s status are identified quickly and may prevent an emergency room visit or hospitalization.
